To log on, use your account username and password.
To log on
1. In the User Name field, type your username.
2. In the Password field, type your password.
3. Click Sign In. You are logged on and the Home page appears.
If you exceed the maximum number of incorrect logon attempts (configured by your healthcare organization), your account will be locked. If your account is locked, you must wait a designated amount of time (specified by your healthcare organization) before your account can be reactivated.
If you have forgotten your username, click Forgot your username and follow the instructions to retrieve your username.
If you have forgotten your password, click Forgot your password and follow the instructions to reset your password.
If you do not have a Sunrise Patient Portal account and would like to create one, click Create an Account and follow the instructions to create an account.
