Forms that have been completed and submitted by patients can be viewed in Sunrise Patient Portal™ and the Clinical Summary in Sunrise Clinical Manager™.
Using Sunrise Clinical Manager™ Configuration Tools, you must set the PatientFormsTileURL environment profile to the URL of the Sunrise Patient Portal™ web site. This environment profile is found in the following location:

After you have configured these 2 prerequisites, forms that patients have submitted will display in the Forms tile of a Clinical Summary view.
Use the Clinical Summary tool in Sunrise Patient Portal™ Configuration Tools to create a Forms tile and add it to a Clinical Summary view. The Forms tile must be of the tile type Patient Forms. In Sunrise Patient Portal™, you can view the forms a patient has submitted by placing the patient in context and navigating to the Forms web part. For more information about using the Forms web part, see the Sunrise Patient Portal™ User Guide.
To view patient forms in Sunrise Clinical Manager™, you must create a Forms tile and configure it to display in a Clinical Summary view using Sunrise Clinical Manager™Configuration Tools. You must also configure the PatientFormsTileURL environment profile using Sunrise Clinical Manager™ Configuration Tools.
